Job Description:
Collaborates with the interdisciplinary team, the patient/family/caregiver, and the patient's primary care provider to develop a holistic plan of care focused on achieving the patient/family physical therapy goalsInstructs, coaches, guides, and intervenes to support the patient and family in achieving the best possible outcomes while remaining in the communities they valuePromotes optimal health, well-being, and quality of life of home health patients, and their families by providing skilled, intermittent physical therapy in the patient's homePractices independently in the patient home using excellent, evidence-based practice for assessment, clinical decision making, clinical practice, and coordination of care across the continuumActs as case manager and team lead for patients requiring home health therapy services, when nursing is either not involved in the case or physical therapy is the primary patient needAssesses the patient and collaborates with the interdisciplinary team and primary care provider to develop the patient-centered plan of careProvides physical therapy treatment to the patient/family/caregiver as orderedCompletes accurate clinical documentation to effectively support service provided within specified timeframes Provides physical therapy to pediatric, adolescent, adult and geriatric patients based on principles of growth and development and life stagesProvides instruction to the patient/caregiver and revises the treatment plan as appropriateMaintains communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methodsReports acute medical problems and condition changes to the primary care providerProvides supervision to Physical Therapy Assistants, Home Health Aides, and students in the clinical settingCompletes regulatory required assessments within specified timeframesCompletes medication reconciliation at specified timeframes
Qualifications:
1-2 years outpatient rehabilitation settingAt least two (2) years of physical therapy experience highly preferredOne (1) year experience in home health or other health setting with geriatric emphasisCurrent license in Physical Therapy or ability to obtain prior to hireAmerican Heart Association HealthCare Provider BLS preferred upon hire, and required within 90 days of hireCurrent Driver's license Excellent customer service skills Ability to work without direct supervisionProvides care appropriate to the age of patients served on the jobKnowledge of physical therapy assessment and treatment methods Knowledge of physical therapy modalities and equipment Ability to maintain effective relationships with other health care professionals Must be able to develop skills in use of technology used in home care settingDemonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff membersAbout Us:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
